#1 Improve Accuracy of Patient Identification

Prior to treatment, involve the client and family in the identification/matching process.

Use 2 client identifiers when giving meds and collecting blood or other specimens for testing –ID bands and photo in Inpatient and DOB & Name in OP.

Label specimen containers in front of the client.




#7 Reduce the Risk of Health Care Associated

Wash your hands and utilize hand antiseptics often.

Follow Standard Precautions at all times.

Educate clients on infection control measures including frequent hand washing.




#8 Accurately Reconcile Medications across the Continuum of Care

Complete a list of the clients’ current medications at intake.

Compare those to the medications ordered by Apalachee staff.

Reconcile any discrepancies.

Communicate up-to-date reconciled med list to the client and next provider of care.




#15 Identify Safety Risks Inherent in the Patient Population

Assess each client for suicide risk factors.

Address immediate safety needs including the most appropriate setting for the client.

Provide crisis hotlines and other prevention resources to clients and their families.

Complete Apalachee Suicide Protocol for all at risk clients.
